I, the undersigned, request permission to participate in the activities of Adventuring, a corporation organized under the laws of the District of Columbia.

I understand that the activities sponsored by Adventuring are cooperative in nature and are initiated, organized, and/or "conducted" by individual volunteers who do not hold themselves out as experts or professionals, and I agree to participate on that basis.

I also understand that certain risks and dangers, both foreseeable and unforeseeable, are inherent in the nature of the activities sponsored by Adventuring, which routinely involve strenuous physical activity in relatively remote outdoor locations, as well as transportation to and from those locations, and I hereby assume all risks of injury to my person and property that may be sustained in connection with my participation in the activities sponsored by Adventuring.

I agree that I will carefully consider my own skill, knowledge, experience, age and physical condition before participating in any activity sponsored by Adventuring, and I agree that I will not participate in any such Activity as to which, any of the foregoing, or other factors, would prevent me from participating in a safe manner and without creating a hazard to myself or to the other participants in the activity.

I further agree that it is my own responsibility to obtain and wear effective safety or protective gear appropriate to such activity, and I agree that I will not use any medicine, alcoholic beverage, controlled substance or other agent that could diminish my alertness or reasoning ability or otherwise interfere with my ability to participate in any such activity in a safe manner.

In consideration of the permission granted me to participate in the activities of Adventuring, I hereby, for myself, my heirs, legatees, executors, administrators and assigns, release and discharge Adventuring and its servants, agents, officers, directors and officials, and all other participants in such activities, from all claims, demands, actions, and causes of action of any sort for injury sustained to my person or property during my participation in the activities of Adventuring.

If I am under the age of 19 years, I certify that I have the permission of my parent(s) or guardians to participate in the activities of Adventuring, and I certify that my own attendance and participation in the activities sponsored by Adventuring is entirely voluntary.

Photography request and release: I, the undersigned, if so indicated, hereby consent without compensation, that all photographs taken of me on the above trip may be used by Adventuring for promotion on the website, adventuring.org, and at other Adventuring functions for promotional purposes only. I understand that the Photographer(s) remain the sole owner of the photographs, and that no financial profit will be made by Photographer(s) of my image without my written consent.

Photographer instructions: For possible posting on Adventuring’s website, resize four (4) of your best pictures to about 640 x 480 pixels and email them to your Trip Leader above. If the photos depict participants, send only photos of people who have signed this form.  The Trip Leader may choose for posting to Adventuring’s website up to four (4) photos from all submissions received from photographers.
